We are accepting new patients from newborns to young adults. Here is everything you need to know before your first visit.
How to get started.
Request your first appointment
Click Schedule Now or call (985) 449-7529. Let us know your child's name, age and whether the visit is for wellness or a specific concern.
Complete your new patient forms
We will send you the intake forms to fill out before your visit. You can complete them online or print them and bring them with you. Doing this ahead of time means more time with your provider at the actual visit.
Bring these to your visit
Photo ID for the parent or guardian, insurance card, immunization records if available, a list of current medications, allergies and prior medical history, plus any school or daycare forms you need filled out.
Show up and relax
We will take it from there. Plan on 30 to 60 minutes for a first visit so we have time to get to know your child and answer your questions.
Your new patient forms
Complete online in about 10 minutes, or download and bring the printed packet with you.
The online form uses a secure signature pad. Sensitive items like Social Security numbers and insurance card copies are collected in person at check-in.
What your first visit feels like.
You will check in at our front desk. Depending on the visit, you will be shown to either our castle room for wellness visits or our ocean room for sick visits. One of our nurses will get vitals, ask a few questions and walk you to an exam room. Then your provider, Dr. Boudreaux, Catherine or Hayley, will join you. The visit is yours. We will not rush you.

A few things to know before you come in.
- We are located at 142 B Rue Marguerite in Thibodaux. Easy parking is available right in front.
- Office hours are Monday through Thursday 8:00 AM to 4:30 PM and Friday 8:00 AM to 3:00 PM.
- For urgent after-hours and weekend concerns, call our main number and you will reach our answering service to connect with a provider on call.
